Transcript Request Form
Use the form below to request to have your transcript emailed to you, to pick it up in the office, or to have it sent directly to your institution of choice. This form is for current students and Class of 2021, 2022, 2023, and 2024 graduates. All prior graduates must request a transcript from BISD's Student Services.
